Notice for Athletes Intending on Competing at Nationals

To all Judo Ontario Athletes intending on participating at the 2013 National Championships:

 

Please read the letter and attachments in this package very carefully.  They provide the forms and information you will need to register for the 2013 National Judo Championships.

5 - Athlete Agreement

6 - Medical Form and Waiver

 

Print the documents, complete them and attach a cheque payable to "Judo Ontario" for the deposit amount.  Please bring all in an envelope marked with your name to the Ontario Open.  Team Ontario staff will review the documents and answer any of your questions.

 

You must meet criteria set out in the Athletes' Handbook for selection on the Provincial Team.  Please note that your spot on Team Ontario is tentative and pending review by the High Performance Committee to confirm that you have met all the requirements.
